Once you have determined that you would like to contact my office for help, please complete the following steps:

  1. For immigration matters, print and fill out the Immigration Privacy Release Statement, giving permission for my office to look into the matter on your behalf. You may also contact my office to request that this form be emailed, faxed, or mailed to you as well.
  2. For Medicare matters, print and fill out the Medicare Privacy Release Statement, giving permission for my office to look into the matter on your behalf. You may also contact my office to request that this form be emailed, faxed, or mailed to you as well.
  3. For all other matters, print and fill out the Privacy Release Statement, giving permission for my office to look into the matter on your behalf. You may also contact my office to request that this form be emailed, faxed, or mailed to you as well.
  4. Gather any necessary documentation and background information that may help my staff better understand your issue.

All Casework inquiries are handled out of my State offices.  It is therefore best to mail or fax copies of any information relating to your request for assistance directly to my state offices. 

Once all the necessary information has been collected, my office will be able to contact the appropriate federal agency on your behalf and initiate a Congressional inquiry.

Privacy Authorization
